While Butch uses neopronouns, he tends to only use them when talking with other queer people. He's more conservative with the trans* aspect of his identity than he is the gay aspect. In that sense, he's stealth.
Born during World War 2, Butch spent much of his early childhood alongside his mother whilst his father served various tours overseas. When the war ended, and Butch's father was discharged due to his mental health, the young child had a fairly average upbringing. Pensions from the military service, and his mother's work in Nashville, were enough to provide the family a comfortable life, and encouraged Butch to pursue his passions as he grew into his teen years. He graduated as the valedictorian of his high school, and briefly went on to begin higher education.
It was during this time that Butch came to realize his gender identity, which would inevitably be his downfall when he began to return to the States.
Rising tensions between nations would lead to a rise of anti-queer sentiments, barring many from finding jobs and community. Having begun the process of medical transition and socially transitioning, Butch found it nearly impossible to do anything with the degree and medical experience he had gotten. This was when he heard rumors of a private mercenary group recruiting experienced individuals for their cause. Forging paperwork for himself, Butch joined Diamond Dogs in 1977, two years after the Ground Zeroes Incident, under the employment of Ocelot. As the organization was established, Butch was transferred to the organization's new, primary base, and would serve permanently as their chief medical officer and on-call combat medic.

PHANTOM PAIN INCIDENT
Butch's role throughout the rise of Diamond Dogs remained fairly straightforward. Patch up those who needed it, train others who showed promise within the field, and make sure requisitions were distributed fairly.
At some point out in the field, Butch would be captured by enemy forces and interrogated for information. He refused to speak, and his "blind loyalty" would lead to the loss of his eye, the engraving on his face, and the loss of his arm. He would escape custody, and would become a wandering soldier for an indeterminate period of time until Snake found him in Afghanistan and retrieved him. Forced to recover from the events, Butch was questioned by the figureheads of Diamond Dogs about the event. Swearing his loyalty, and that no intel was leaked through the event, he was placed under direct supervision as to ensure no double-crossing occurred. Once cleared, work resumed as normal.
Things would only continue to turn on their head, though, when a mysterious illness began to take over the base.
From there, Butch's focus became on managing and aiding the Quarantine Platform established by Snake. Word was eventually received that the group had found someone who would know how to address the outbreak, and Butch worked closely with the man, named Code Talker, to accomplish this task. Inflicted patients were handled on a case-by-case basis, with many receiving treatment and recovering (though with the caveat that many would become infertile). Still, Butch was more than happy to see that lives could be saved... Until it happened again.
This time, though, the situation was worse. Much worse. A radiation leak had caused a mutation in the medications used to suppress the parasites, thus mutating them and making them able to clone without sexual reproduction. Butch was stuck on the platform when Snake was sent in, and is one of few who survived the horrowing events that followed his arrival.
After the fact, Butch had little time to recover and process the mass casualties the forces experienced. The war was still waging, and he still had a job to do. Still, he did well to check in with those above and beneath him, offering what aid he could with the resources they had.
Later on, as the battle against XOF continued, Butch was dispatched among the units of Diamond Dogs to move in on the aftermath of the Sahelanthropus incident in Central Africa. He was the medic who ran in to evaluate Snake, and in the process the medic who confirmed that Eli hadn't died in the battle thanks to the bulletproof vest he'd donned. Venom Snake ordered that he be returned to Mother Base to be patched up, but as Eli began to come to, he began showing symptoms of the English vocal cord parasite's effects. Eli expressed his hatred for Snake and Cipher, claiming that someday he would destroy Big Boss, believing Venom Snake to be his father. With the island completely contaminated by the vocal cord parasites, Ocelot reminded Snake that they had to leave soon and then ordered the entire island to be hit with a napalm strike to burn every last thing. As Butch, Diamond Dogs, and the salvaged remains of Sahelanthropus began to move out, Snake raised his pistol to Eli's head to kill him in an act of mercy, rather than let him die from his parasite infection or the bombing, but found himself unable to pull the trigger on the kid, whom he had come to respect as "one hell of a soldier." Snake instead left his gun with one shot in it on the ground in front of Eli, telling him "Don't blame yourself. Blame me," before he turned his back on him and walked to the nearest helicopter, leaving Eli with the decision of what to do. On board the Diamond Dogs helicopter, Venom Snake and Ocelot overlooked the bombing of the island. This incident would truly mark Butch's loyalty to Venom Snake's command, as even after he would continue to follow his order.
OUTER HEAVEN UPRISING
After completing the construction of Outer Heaven's fortress in South Africa, Big Boss's reputation as a mercenary had travelled far and wide thanks to the activities of Venom Snake. While he knew that mercenaries around the world would eventually rally to his cause, Big Boss also knew that it would bring the attention of government agencies growing suspicious of his activities. Needing to keep attention away from Outer Heaven, Big Boss himself decided to return to the U.S. to rejoin FOXHOUND as its commander with Venom Snake taking control of the operation in his absence. Like many Diamond Dogs, Butch would follow, establishing his base of operations within Outer Heaven. The growing suspicions from the world, though, would lead to Butch questioning whether or not he was truly fighting for the right cause anymore.
This was especially true when the Outer Heaven Uprising began in 1995. Hearing of Snake's forcing of the scientist Drago Pettrovich Madnar to build TX-55 Metal Gear by holding his daughter, Ellen, hostage, Butch began making attempts to distance himself from the organization and look for a way out. Unfortunately for him, Solid Snake's work proved too successful, and the base's self-destruct sequence was activated. Shut in, Butch was one of many casualties in Big Boss' plan to fake his death and cover his tracks.
